**Our appointments are in high demand. There is a $50 fee for scheduling online and not showing up for the appointment. Please do not book if you’re not coming.
If you would like to know your insurance benefits prior to scheduling, please contact the customer service number on the back of your insurance card.
*As of January 2025, we are no longer accepting Community Health Choice Medicaid, Any Medicare or Any Marketplace Plans.
Before Booking:
- New Patients are individuals who have never been seen by the doctor.
- Existing, Active Patient : has been seen by the doctor and is currently under care
- Existing, Inactive Patient has not been seen in 3 months or more. Your online profile may be inactive. Please do not schedule as a new patient or change your name, phone number or email in order to book. This creates a duplicate patient account and any appointments scheduled will be canceled or rejected by the system. You will need to call to book an appointment.
